I, the undersigned parent or legal guardian of the child or children listed above, do hereby assume all risk of injuries, possibly minor, or serious, or catastrophic in nature, that may result from participation in activities related to the Brookwood Carnival, whether allegedly resulting from my child’s negligence, or the alleged negligence, actions or inactions of the Brookwood parents, teachers, staff and the association (collectively defined as the “PTA”). Acting for myself, my children, my heirs, my spouse, personal representatives, and assigns, I do hereby release, waive, discharge and covenant not to sue the PTA or the owners or lessors of the carnival premises, from and with respect to any and all liability, including claims and suits at law or in equity for an injury that may result from participations in or use of Brookwood Carnival games and rides. By allowing my child to participate in the event, I assume all foregoing risks and accept personal responsibility for the damages following any such injury or permanent disability.
As legal parent or guardian of the child, I do hereby verify by checking "I agree" and submitting this online form that I have read and fully understand and accept each of the above conditions for permitting my child to participate in the Brookwood Carnival activities, including the carnival games and rides.
